What to bring to your medical appointment
Arrive 20 minutes before your scheduled time and complete the check-in process.
Bring your medical order, authorization from your insurer, medical history from your referring doctor, and any laboratory tests or diagnostic images.
Keep in mind the reason for your visit so you don’t forget any important details during your consultation.
After your appointment, claim your medical record at Window 4 (Medical Records), located on the first floor of the Clinic.
If you are a private patient, you have 15 days to access a follow-up appointment at no additional cost.
